File a Claim

Health Insurance Claims

Most of the time, your medical service provider will submit the claim on your behalf. However, for those times you actually need to make a claim, you may do so using any of the following methods:

Email

Scan your receipts (with diagnosis codes) and email them to Colonial Medical noting your name and certificate number.

Walk-in

Bring in your receipts (with diagnosis codes) and your Medical ID card to our third floor reception desk at Jardine House on Reid Street. We will take a copy for you and will mail your cheque when it is ready.

Mail-in

Send a copy of your receipts (with diagnosis codes) noting your name and certificate number to Colonial Medical at P.O. Box HM 1559, Hamilton HM FX. Please be sure to include a contact number in case we have any questions.

Don't forget, you can view your claims history on the Medical Web Portal. If you haven't signed up yet, please contact us.

Life Insurance Claims

Please call a Customer Service Representative on 293-LIFE (5433) or come in for assistance.

Business Insurance Claims

Please call a Claims Officer on 296-3700 or come in for assistance.

Home & Motor Insurance Claims

Please call a Customer Service Representative on 296-3700 or come in for assistance.

Motor accidents or thefts should be reported to our Claims Department as soon as possible and certainly within 14 days, even if you feel you will not need to make a claim.

Property damage or loss should be reported to our Claims Department as soon as possible and certainly within 30 days.

For both motor and property claims, please do not  have any repairs made until we have been notified and the damage has been inspected.
